High Blood Sugar and Diabetes

High blood sugar and Diabetes are the leading cause of kidney failure, blindness and lower limb amputation. High blood sugar also increases your risk of having a heart attack or stroke. Having uncontrolled high sugars can cause these complications earlier.  Lowering your blood sugar level reduces the risk.  The ACTIVE Diabetes Care Team helps patients with difficult to control diabetes reach their personal health goals.  Working with your doctor, our Board Certified Nurse Practitioner, Clinical Pharmacist, and Nutritionist offer one-on-one extended visits to our patients and families to help you better understand:
 
  • Diabetes – what it is and how it affects the body
  • Nutritional awareness – the importance of diet and making good choices
  • Active living - why it is important to exercise or be active
  • A balanced approach to health
  • ​Identify and focus on individual challenges to reaching personal health goals

Special Care Center –

Changes in your health can be confusing, especially after a stay in the hospital or a nursing home. We want to make sure that you have the right medicines, appointments, and support.  Our team of Board Certified Doctors, Nurse Practitioners, Clinical Pharmacists, and support staff at our Special Care Center are there to help as you head back home. 


The Anticoagulation Center, Special Care Center, and The ACTIVE Diabetes programs are located in our new Fountain Valley office.

SPARK!


SPARK is a voluntary Greater Newport Physicians program offered to certain patients as directed by their doctor.  The SPARK specialized care team includes your primary care physician (PCP), care managers (nurses and other professionals) and support team members. The care team will help you coordinate health services, navigate the health care system, and help you to improve and maintain your health.  The goal of the team is to help you find solutions to issues and improve your health and well-being.
